6. Did you experience any additional problems after the surgery or need any additional revision surgeries?
7. What is the name & address of the hospital(s) where your surgery/surgeries were done? (remember this will not affect the doctors or hospitals)
8. If a female, were you pregnant at the time of the original implant?
9. Do you recall the manufacturer of the implant you had inserted?
